Effective September 18, 2026
Pervia Medical Transport, LLC provides scheduled wheelchair and ambulatory medical transport across southern Dallas County, Texas. This policy explains what happens to information you give us through this website.
If something here is unclear, call dispatch at 469-568-6670 and ask.
The short version
This website asks for as little as it possibly can. The transport request form collects your name and phone number, and nothing else. We do not ask for your diagnosis, your medications, your appointment, your pickup address, or where you are going. Those details belong in a phone call, not in a web form, and we designed the form that way on purpose.
We do not run advertising trackers. We do not sell or rent anyone's information. There is no cookie banner on this site because there is nothing to consent to.
What we collect, form by form
Request transport
Two fields:
- Your name
- A phone number we can call you back on
That is the entire form. We use it for one thing: to call you back and arrange the trip. Everything else — when, from where, to where, what assistance you need, and what it will cost — is discussed on that call.
Training and certification enquiries
If you ask about CPR or life-support training for your staff, we collect your name, your organization, your email address, your phone number, and what course, how many people, and roughly when. This is business scheduling information about an employer, not health information about a patient.
Careers
If you apply to drive or instruct for us, we collect your name, email address, phone number, the role you want, your city, a short description of your experience, and a résumé if you choose to attach one.
What we do not collect on this website
We do not ask for, and the forms on this site cannot transmit:
- Any diagnosis, condition, medication, or treatment
- Medical record numbers, insurance or Medicaid ID numbers
- Social Security numbers
- Pickup or destination addresses
- Appointment dates, times, or facility names
- Payment card numbers
If you type clinical details into a free-text box anyway, please don't. Call us instead. We will handle anything that reaches us appropriately, but the safest place for that information is a phone line.
This is a website privacy policy. It describes what this site does with the information you type into it. It is not a HIPAA Notice of Privacy Practices, which is a separate document governing protected health information created in the course of providing transport. Ask us for that document directly.
How we use what you give us
We use form submissions to respond to you — to arrange a trip, quote a class, or consider an application. We may keep a record of the contact so that we can follow up, honor a quoted rate, or show that a request was handled.
We do not use this information for marketing to people who did not ask to hear from us, and we do not add you to a mailing list because you requested a ride.
Where it goes, and who sees it
When you submit a form, two things happen. The submission is saved in this website's database, and a copy is emailed to info@perviamedicaltransport.com, which is monitored by Pervia staff.
The people who can see it are the Pervia personnel who need it to do the work — dispatch, the owner, and, for applications, whoever is hiring. We do not share submissions with data brokers, lead-generation companies, or advertisers.
We may disclose information where the law requires it, such as a subpoena or a lawful request from a regulator, or where it is necessary to protect someone's safety.
Service providers
A handful of companies handle data on our behalf in order to make the site function:
- Our web host, which stores the site and its database
- Our email provider, which delivers form notifications to our inbox
- Google Fonts, which serves the typefaces this site uses. Loading a font means your browser makes a request to Google, which reveals your IP address to them. We use no other Google service — no Analytics, no Ads, no Tag Manager.
Payments
If you pay us through this website, the payment itself is completed on a page hosted by our payment processor, not by us. Card numbers are never entered on, transmitted through, or stored on this website. We see that a payment was made and for how much; we never see your card details.
Payment references are kept deliberately generic. A payment record shows an amount and a name — it does not describe a medical service.
Résumés
A résumé you attach is stored on this website's server under a randomly generated filename that is not guessable and is not linked from anywhere on the site. That makes it effectively private, but it is not password-protected. If your résumé contains information you would rather not put on a web server — a home address, a date of birth, a license number — remove it and bring it to the interview instead.
Spam protection
To stop automated form abuse, the site briefly notes how many submissions have come from your network connection. Your IP address is not stored as-is: it is converted into a scrambled value that cannot be read back into an address, used only to count submissions, and deleted automatically after one hour. It is never attached to your name, your submission, or anything else.
How long we keep things
- Transport requests: retained while we arrange and complete the trip, and for a reasonable period afterward for service and billing records.
- Training enquiries: retained while we follow up, and afterward as an ordinary business record.
- Applications and résumés: retained while the role is open and for a period afterward in case another role fits. Tell us to delete yours and we will.
- Spam-protection counts: one hour.
Cookies
This site sets no advertising or analytics cookies. WordPress sets a login cookie for Pervia staff who sign in to administer the site; it is not set for ordinary visitors.
Children
This website is not directed at children and we do not knowingly collect information from them. We do transport minors as riders — those arrangements are made by a parent, guardian, or facility by phone, not through this site.
Your choices
You can ask us what we hold about you, ask us to correct it, or ask us to delete it. Email info@perviamedicaltransport.com or call 469-568-6670. We may need to confirm who you are before we act, and we may need to keep records the law requires us to keep.
Not submitting a form is always an option. Calling dispatch directly works just as well and leaves less behind.
Changes
If we change this policy we will post the revised version here and update the effective date above. Material changes to how we handle information will not be applied retroactively to information already collected.
